Background

 

After allogeneic hematopoietic stem cell transplantation (allo-HSCT), the donor-recipient chimeric status can be monitored dynamically to access the success of transplantation, the occurrence of graft-versus-host disease (DVHD) disease recurrence, and graft rejection, thereby guiding corresponding early clinical intervention treatments. Short tandem repeat (STR) is a highly polymorphic tandem array of specific repeats in DNA, and individuals can be accurately identified after PCR amplification. Therefore, STR loci fluorescent multiplex PCR amplification with automated capillary electrophoresis is the most sensitive method in chimeric status detection.


In HSCT, hematopoietic chimera (HC) refers to the partial or complete engraftment of donor-derived hematopoietic cells into the recipient. Generally, chimerism status is categorized as follows: "complete chimerism" (CC) when donor cells occupy > 95% of the recipient's bone marrow or peripheral blood, known as complete engraftment of donor cells; "mixed chimerism" (MC) when recipient cells still appear in the bone marrow or peripheral blood after transplantation, and the donor and the recipient's cell components can be detected together, with 2.5% to 95% of donor cells; and "microchimerism", when donor cells occupy < 2.5%. The decrease in donor chimerism (DC) is closely related to graft rejection and disease recurrence. Studies in China and abroad have confirmed that dynamic monitoring of DC can determine engraftment and disease outcome. This is also of great significance in predicting the occurrence of GVHD, and guiding the subsequent immunotherapy of transplantation and other early clinical intervention treatments.

Specimen Requirements

 

 

Initial specimens: peripheral blood/bone marrow (2–3 mL EDTA anticoagulation, 4 °C, within 48 h for testing); dental cotton, hair, and nails (no contamination)
Retest specimens: peripheral blood/bone marrow (2–3 mL EDTA anticoagulation, 4 °C, within 48 h for testing)
